Abstract
This report summarizes the experiences and concerns of 6 Massachusetts adolescents and young adults with special health care needs (ages 11 to 22) and 30 family members. The study seeks to understand the issues and needs that young adults and their families face during the transition from school to adult life. The results of six focus groups and three detailed case studies illustrate the variety of challenges and solutions that individuals with special health care needs and their families experience.
